On Thursday, three citations totaling $36,750 were issued to Gerdau Reinforcing Steel West in connection with the death of a company truck driver at the new San Francisco 49ers stadium in Santa Clara, California, in October 2013.

The California State Division of Occupational Safety and Health (OSHA) issued the fines, saying that Gerdau failed to implement procedures to verify truck drivers and other employees were safe while the truck was being unloaded.

The victim, Edward Lake II, was crushed by a bundle of rebar that fell from his truck—the second worker to die during construction of the $1.2 billion stadium.
